Your First Visit
Please arrive at the Stabler Clinic, PC at least 15 minutes early for
your scheduled appointment. You will be asked to complete health and
insurance related forms. Please bring any insurance cards or papers
with you on your first visit. After your initial visit, please inform
the receptionist if you have any changes to your name, address, phone,
marital status, employment or insurance carrier.
Wheelchairs are available for patient use while at the Stabler Clinic,
PC. They may be obtained at the main entrance.
Please
bring a list of your healthcare concerns to your appointment. Also
bring the medications you are taking, including prescriptions,
over-the-counter drugs, vitamins and nutritional supplements. It may be
helpful for you to develop a family health history of close blood
relatives. Tell the doctor about any pertinent changes in your
lifestyle and about the nature and frequency of your symptoms. |
